Tell me about your bad dates!

Hey there,
My name is Lisa and I'm a freelancer with a national magazine looking to collect 500 stories about bad dates in New York. I'm not supposed to name the mag in a public forum, but of course, I'm happy to share all details if you get in touch directly.
What I need: The tale of your worst date in New York. Nothing too long or involved, and you don't have to spend any time writing it with style. There'll be editing, after all. It could be as simple as a sentence—"I liked him so much that I invited him over after our first date, but then he puked in my bed!"—or a 100-word summary. Anything will do, so long as your story is real.
One other thing: The magazine wants to publish your name, age, and occupation. I know this may sound scary, but just think of it this way: There will be 500 of these in the magazine. It's not like your name will be gigantic and prominent. And it makes your story all the more real—readers know it's true when there's a name attached.
So! I'd really, really appreciate it if you sent me your story, along with:
Your first name and last initial
Occupation
Age
Phone number (just for fact-checking purposes—it won't be published!)
I need to do this by Friday Feb 3rd, so any and all help would be greatly appreciated.
