Need a simple HTML form created ASAP

Need a simple 1-page HTML form. I'll just need the code so I can throw it up on a site. Will require a couple of field-dependent options, so will need a little Javascript. It's easy, I swear, but I'm a n00b. It would probably take 20 minutes for an experienced person with an editor (or so I think -- feel free to school me). Will need this done in the next 48 hours.
